For the family reunification visa with a EU citizen, the following documents are required:
1) Visa application form (can be picked up at the counter or downloaded from the Consulate’s website)
2) Recent passport-sized photograph (it is advisable to bring a second photograph in case a DNA Test is required)
3) Valid travel document with an expiration date at least three months beyond that of the requested visa
4) Copy of the identity document of the inviting EU-Citizen;
5) Documentation proving the status of family member or spouse of the foreign citizen residing in Italy.
The documents to be submitted to prove the status of family member as per point 5) are:
For all applicants:
- Birth Certificate (if born after December 1, 1992) or a Sworn Declaration of Age and Attestation of Birth issued by the National Population Commission.
For those reuniting with a spouse:
- Marriage (in the event that the marriage has already been registered in Italy, a substitute declaration in accordance with laws 445/2000 signed by the Italian citizen will be accepted)
- Affidavit of name change (only for married women, if applicable).
For minors:
- Consent for visa issuance from the parent remaining in Nigeria (if applicable) in the form of an Affidavit. The consent document must also be legalized and translated, and a copy of the identity document of the declarant must be attached. If the declarant is in Italy, it is necessary to proceed with the signature authentication at the municipality of residence. N.B. The declaration must explicitly mention the consent to live permanently in Italy with the other parent.
- In the event that the other parent has passed away, a death certificate of the other parent. For those reuniting with children living in Italy: a) Birth certificate of the child (if absent, a DNA test will be required)
All documents listed in point 5) must be legalized and translated into Italian. The cost for the legalization for each document is 38,400 Nigerian Naira and for each certificate of translation conformity, it is 20,800 Naira (13 Euros).
N.B: For children reuniting with parents or parents reuniting with children, in case of doubts about the authenticity of the documents submitted, applicants may be required to take a DNA test. In that case, an additional form will need to be filled out (downloadable here or provided at the time of visa application submission).
In case of reunification with an Italian citizen spouse, the transcription of the marriage in Italy must be requested, if it has not already been done.
